Arizona Department of Corrections Rehabilitation and Reentry Application to Visit an Inmate It is important to complete this application and answer all related questions truthfully.  An improper or incomplete application shall not be processed. All information is considered confidential. The inmate will be notified whether the application is approved or disapproved. It is recommended all applicants review Department Order 911, Inmate Visitation   prior to completing the application.

If you would like to visit an inmate, please complete the Visitor Information Section of this form, sign and date the application. For Minors: If this application is for a child(ren) under the age of 18, a non-incarcerated parent, legal guardian, or temporary custodian of record shall complete, sign and date an application for each minor. If the non-incarcerated parent, legal guardian, or temporary custodian of record authorizes another adult to bring in the minor(s), that adult shall be an approved visitor. A notarized letter shall be provided by the non-incarcerated parent, legal guardian, or temporary custodian of record authorizing the adult to bring the minor in to visit an inmate. This letter shall include the ADCRR number and inmate name. A SEPARATE APPLICATION MUST BE COMPLETED FOR EACH PROPOSED VISITOR (ADULT OR CHILD).
